A Credit Card or Debit Card is required to be kept on file with us for all transportation services. We don’t accept American Express at this time.

By filling out this form, you are giving us, Astoria Express Transit permission and authority to transport your child to and/or from the location you listed above. You agree and acknowledge that all payments are due IN ADVANCE AND BEFORE SERVICE IS RENDERED. For future invoices, if payment is not received on time, a $10 late fee will be added to your invoice. You also agree Astoria Express Transit is not responsible for any lost or stolen items on one of our buses. Astoria Express Transit also reserves the right to cancel your child’s service and/or recoup any fees ASSOCIATED WITH DEFACEMENT OR VANDALISM THAT YOUR CHILD HAS CAUSED, MISSED PAYMENTS OR MISBEHAVIOR FROM YOUR CHILD. If you fail to comply and agree to these terms of service, we cannot transport your child.
